Date of Birth & Social Insurance Number verification

Verification of Experience

If you have previous experience as a teacher on contract, call and request a Clearview Public Schools Verification of Experience form or submit documentation for evaluation.

Current Criminal Record Check – Vulnerable Sector

Obtain at police station or RCMP station: must be satisfactory and recent (within 6 months); please send original – we can make a copy and return original upon request.

Intervention Record Check (aka Child Welfare Record Check)

Obtain from a Provincial Child Welfare office; must be satisfactory and recent (within 6 months); please send original – we can make a copy and return original upon request.

Teacher Qualification Statement (TQS) – TQS Application form

​Statement of years of training, issued by the Alberta Teachers’ Association (1-800-232-7208) please send original, we can make a copy and return original upon request.

Needs to be signed in order for us to list you as a teacher or substitute teacher. Please leave BLANK any lines you feel you don’t want to share information on.

For Substitute Teacher application packages please submit all of the above and indicate:

Please indicate in your personal information whether or not you are currently receiving from TRF (Teachers Retirement Fund).

If you are receiving a Canada Pension Plan retirement pension, you must provide proof (Notice of Entitlement) from CPP indicating the start date of your pension to our Human Resources Department.

The Substitute Teacher list is compiled and updated at Division Office and sent out to schools periodically. Booking of Sub Teachers is done directly from the school from the approved list only.
