We are currently accepting applications for the role of Care Manager. This position is highly Sales Driven and focused on Customer Service. The opportunity for growth is endless!

THE ROLE

The Care Manager will collaborate with the Branch Team while generating health care referrals through relationship- building and maintenance with key referral partners and stakeholders in the health care industry. Opportunities abound to seek out new business relationships and opportunities thorugh marketing and cold calling strategies. The use of social media tools and prospecting techniques are integral to this role. The Care Manager will provide care assessments in the home, building and nurturing relationships with clients in the community. A key component to this role will be the ability to build our brand and our client-base. Collaboration with creation and delivery of your Clients Care Plan as well as leadership with the integration of client satisfaction initiatives and managing client concerns in a timely manner.

Job Qualification

QUALIFICATIONS

Education

Graduation from an educational program focusing on geriatric care, community health or social work.

Experience

At least 1 year of recent experience with responsibilities for client acquisition, care management and business growth, preferably in a community setting.

Other Skills and Abilities

Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format; ability to work independently and effectively lead a team; above average report writing and communication skills; valid driver’s license; solid knowledge of the principles, practices, and methods of customer service, care management as well as program development, implementation and evaluation.

REPORTING RELATIONSHIPS

The Care Manager reports to the Area Director. Care Manager will receive support from the Sales Coach as necessary and related to position accountability.

QUALIFICATIONS

Education

Graduation from an educational program focusing on geriatric care, community health or social work and/or an equivalent combination of education and experience. Related education or experience in business or marketing is considered an advantage. Previous sales training is considered an asset

Experience

At least 2 years of recent experience with responsibilities for client acquisition, care management and business growth, preferably in a community setting. Experience developing referrals and able to network within the assigned territory

Other Skills and Abilities

The ideal candidate is highly motivated, results driven and able to develop and build strong business relationships within the assigned territory. Demonstrated ability to meet sales targets,

ability to develop and implement a sales and marketing plan. Excellent planning, organization and communication & presentation skills are essential
