Description
Hours:
- Casual/on-call, Monday to Friday, 8:00 am – 4:00 pm
KEY RESPONSIBILITIES:
- Conducts program referral assessments and interviews
- Develops, implements and monitors personalized case plans for assigned program residents
- Coordinates and facilitates residents personal, educational / employment programming
- Monitors residents’ activities, program compliance and evaluations
- Maintains case files and detailed progress notes
- Participates in case management meetings
- Provides coaching, advocacy, emotional and spiritual support to residents
- Consistently demonstrates empathy and concern for residents through cooperative, collaborative relationships
- Assists in providing/facilitating life skills training sessions
- Performs resident intake, discharge and long term follow-up, random drug/alcohol testing
- Provides ongoing information for building lasting community support networks to assist resident with social and community reintegration
- Liaises with referral sources and other external agencies as needed (probation, Parole, M.S.D.S.I., treatment facilities, etc.)
- Provide program presentations to various groups and organizations
- Contributes to and responds to program evaluations to adapt and improve program content, develops program initiatives
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
- Diploma or degree in social services
- Minimum three to five years of prior related experience
Successful candidates, prior to hiring, may be required to provide:
- Background check consent
- A clear vulnerable sector screening
- A clean drivers abstract
- Completion of our online Armatus Abuse Training and required Health and Safety training
