Description
The caseworker will support clients and families both through regular intake and particularly through case management of Pathway of Hope (POH) participants, seeking to identify and partner to address root cause issues with clients. The case worker will use the POH approach to assist families in moving from crisis to stability, networking with other local organizations as necessary. The successful candidate will assist in implementing this program to our ministry.
Hours:
- Full-time – 40 hours per week.
KEY RESPONSIBILITIES:
- Support client intake at the food bank while assessing for possible POH candidate readiness
- Engage and build rapport with clients, families and guests
- Perform all aspects of the case management process using the POH approach(selection, intake, assessment, goal planning, action, transition, and follow up)
- Develop goal plans with client families and schedule regular meetings to review progress and engage in further planning
- Maintain case files for each client family, including accurate documentation, detailed case notes, and use of all appropriate forms, ensuring confidentiality is maintained in accordance with operating policy and legislation
- Provide information and referral services as needed, for any client seeking assistance
- Support with seasonal services (ie: intake for Christmas assistance or summer camp)
- May conduct home visits as needed
- Network with other services providers and maintain working relationships with community agencies to provide comprehensive services for participants
- Attend agency and community meetings as needed
- Work closely with other local ministry unit personnel to form an interdisciplinary team to work with families from a holistic perspective
- Attend monthly pathway of hope (POH) training meetings (in-person/virtually)
- Attend training sessions that support the initial implementation phase and ongoing training for the sustained delivery of POH
- Ensure accurate collection and maintenance of data, submit monthly summary of service statistics to the regional coordinator, and assist the regional coordinator in semi-annual evaluation/outcomes measurement.
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
- Completion of a 2-year community college diploma in a human services field(such as Child & Youth Worker, Developmental Services Worker, Community Worker, Social Services Worker, etc)
- Completion of a university degree in a relevant field would be considered a definite asset (ie: Bachelor of Social Work, Bachelor of Arts in a Social Sciences field, such as psychology, sociology, gerontology, or anthropology)
Experience:
- 2-5 years prior related experience, including providing casework services, documenting/maintaining case files, working collaboratively with other service providers, community outreach and working with people of diverse backgrounds
- Competence in developing/maintaining appropriate client relationships and inter-agency relations
- Experience with new/existing program facilitation and administrative duties

- Experience in establishing programs/services an asset
- Flexibility in navigating/implementing change
- Aware of trauma-informed care, non-violent intervention and social determinants of health
- Optimistic and hardworking, ready to work cooperatively in new/existing initiatives
Successful candidates, prior to hiring, may be required to provide:
