Responsible to the Director, Home and Community Care, the Manager, HCC, Program Practice and Education is an integral member of the management team and works collaboratively with clients, physicians, managers and community resource staff in the planning and development of practice and education initiatives.
The Manager provides leadership in establishing standards and policies to support the model of care and quality patient care and in developing and implementing systems for quality improvement in a program management framework.
Provides management, leadership, facilitation and coordination of a interdisciplinary team that support the development, implementation and evaluation of practice strategies, standards and education. Includes overall responsibility for the management of specific projects, including management of related human and financial resources.
The Manager is responsible for all aspects of planning, organizing and directing the staff and activities of the practice resource team ensuring departmental area policies and procedures comply with professional, legal and Vancouver Island Health Authority (VIHA) standards.
Provides leadership in the development and provision of training and education programs designed to attract and retain qualified professional staff to Home and Community Care. This is a strategic priority for VIHA Home and Community Care Program.
QUALIFICATIONS:
Education, Training And Experience
Interdisciplinary Master’s (Nursing, Health Sciences) with 7 – 10 years in Home Health experience including experience in health informatics, quality improvement and clinical documentation systems.
Skills And Abilities
Knowledge and experience in adult education and program development.
Excellent knowledge of issues affecting home and community clients in all areas of service delivery and service delivery structures
The appropriate certifications, licenses or professional designations required by the professional discipline
Able to lead, direct and develop staff in a rapidly changing environment.
Commitment to client focused service (enablement model) and teamwork
Project management, evaluation and community development skills
Excellent oral and written communication skills
Ability to develop and nurture an environment where quality improvement principles are embraced and used on a continuous basis.
Ability to work independently and with initiative
Demonstrated analytical and critical thinking skills
