Description
To provide health education and health advocacy for the young parent families of Bethany Hope Centre (BHC).
KEY RESPONSIBILITIES:
- Build therapeutic relationships with participants to assist with physical and mental health education, support, and resources both within BHC and the community
- Make appropriate referrals as required (both internal and external)
- Advocate as required for those supports
- Coordinate health services for the Centre.
- Liaise with Child Care Centre to determine infant and child development needs
- Monitor infant and child participant needs and progress through various screening tools
- Continue to develop logic model for health services for BHC
- Continue to seek out and support the preparation of funding applications relevant to the health care program
- Continue to develop a smoking cessation program and promote breastfeeding supports
- Perform Mental Health and Wellness checkups with the clients either at the Centre or in their homes
- Develop and maintain Community Partnerships
- Liaison with Ottawa Public Health Sexual Health Nurse
- Coordinate weekly Medical Clinic in association with Bruyere Academic Family Health Team (BAFHT)
- Encourage and administer immunizations where possible
- Keep the Health Centre stocked with supplies needed to carry out care. Replenish as required
- Complete all required documentation including:
o According to the College of Nurses Standards, document all nursing activities into the nFocus system
o For BAFHT patients, enter data into the patient’s BAFHT patient chart
- Maintain statistics
- Sit on relevant committees as requested by the Director of Child and Youth Development or Executive Director
- Meet with supervisor on a regular basis to discuss workplan
- Other duties as required
- Uphold the values of the Salvation Army and the mission of Bethany Hope Centre
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
- Graduate of a recognized School of Nursing e.g. RPN, RN (University degree preferred)
- Current registration with the College of Nurses of Ontario
- Current CPR and First Aid training
- Credentials in addictions and mental health care an asset
Experience:
- Minimum 3-5 years nursing experience
- Experience in community nursing an asset
- Experience working with children and/or young parent families an asset
Required Skills/Knowledge:
- Second language is an asset
- Must be flexible in a fluid environment where programming can change quickly
- Have a heart/passion for the marginalized and a non-judgmental attitude
- Willingness to work within the Christian mandate of The Salvation Army and the mission and policies of Bethany Hope Centre
Successful candidate will be required to provide upon hiring:
- A clear vulnerable sector screening
- A clear internal abuse registry check
- A clean drivers abstract
- Participate in our online Armatus Abuse Training and Health and Safety training required upon hiring, as well as updated annually
HOURS: 25 hours per week
